Faculty members conducting research or teaching at Georgia State University may create website to display their academia related activities. An instruction can be found here.

Travel Forms

Up-to-date Payment Request Form, Travel Authorization Form, and Travel Expense Statement can be found by searching here. Receipts submitted for reimbursement need to include the your name the method of payment, and last four digits of the account number. If the method of payment does not appear on the receipt, a credit card/bank statement showing the your name, transaction, and last four digits of the account number is needed.

Questionnaire and Poll

SurveyMonkey is a website for online polls (or questionnaires). To create a poll, just follow the instruction on this website. You will be the administrator of the poll you created. When you create the poll, you will be asked to provide the email addresses of your target participants (you can upload a spreadsheet such as Microsoft Excel file to contain a long list of emails), and the website will notify them by emails with a link to the poll. Here are some notable features:

1. Each participant is linked to one email address and can only respond once.
2. You can enable auto-response when you create the poll, so that a participant will receive an email confirmation from the website after submission.
3. The poll result can be made completely anonymously (to both you and all the participants) by turning on "Anonymous Response" option. In this case, only the poll statistics can be seen by the poll administrator and participants. The poll administrator can also see whether an email address has responded or not.
